Coordination of Care
When you choose Desert Springs Hospital Medical Center for rehabilitation, you will receive the full continuum of services and care. If you need urgent medical care, it is available to you right here at hospital, just steps away from our rehabilitation center. The multi-disciplinary team includes:
Physiatrist — A physiatrist is a doctor that specializes in physical medicine and rehabilitation. Your physiatrist will meet with you when you are admitted and will work with you to develop recovery goals. Your physiatrist coordinates, manages and monitors your day-to-day medical and rehabilitation needs.
Nurses/Certified Nursing Assistants — Our nurses are experienced with rehabilitation care. They help manage medications and reinforce your therapy goals. Certified nursing assistants (CNAs) assist in building tolerance and completing activities of daily living (ADLs), such as getting out of bed, dressing yourself and setting up your meals. CNAs also transport you to and from therapy.
Physical Therapists — Your physical therapists will focus on coordination, balance and gait training. Your physical therapist will help design a program to help you improve your mobility and strength.
Occupational Therapists — When you improve function, you can return to your highest level of independence. Occupational therapists focus on ADLs, including bathing, dressing and self-care activities. They also identify specialty equipment you may need following discharge.
Speech and Language Pathologists — Our speech and language pathologists treat speech and swallowing disorders and help you enhance your cognition and comprehension, verbal expression, listening, reading and writing, as well as attention, memory and problem-solving skills.
Case Managers — Your case manager acts as your liaison and works closely with you and your family. Your case manager helps establish your discharge plan and sets up home care and outpatient services needed following your discharge.
Respiratory Therapists — Respiratory therapists monitor your oxygen needs and provide breathing treatments and medications needed to build strength and endurance.
Nutritionists — During your rehabilitation, you may need special meals and higher calorie foods to help build your strength and endurance. Our dietician will monitor your nutritional needs and make recommendations to ensure you receive proper nutrition to support your recovery.
Pharmacists — Medication management is important to your recovery. Pharmacists and pharmacy techs answer questions on specific drug and medication effectiveness and manage medications to ensure safe, quality care.
Housekeeping — A clean, safe environment supports your recovery. Our professional cleaning and maintenance staff is available throughout the day and night.
In-house Dialysis — Patients recovering from significant injuries and illnesses may require specialized services, like dialysis. As part of the Desert Springs Hospital Medical Center complement of services, dialysis service is available onsite.
